Below you will find downloadable registration forms, referral forms, a general list of our accepted insurance plans and financial policies, as well as other helpful information.

New Patient Paperwork

There are several ways to request an appointment with the physicians of Amarillo Bone and Joint. You can call Amarillo Bone & Joint at ‍(806) 468-9700 and have one of our admitting personnel assist you.
To facilitate the appointment process, please have your insurance card available as we will ask you about your coverage.

You may come to the office approximately 30 minutes before your appointment time and complete requisite paperwork or you can complete your New Patient Paperwork online or download a copy.

We require that you bring any recent X-Rays, MRIs or CT scans with you to your appointment. You will have to contact that facility where the procedures were performed and sign a release for the films. We would like to have the film and reports. Without those films, we may have to reschedule your appointment so that we do not duplicate diagnostic tests. In some cases, the films or techniques are not adequate and may need to be repeated.

Also, if your insurance plans have office visit co-pay, you will be expected to pay the co-pay at the time of your visit.cm

Financial Policies

The Physicians of Amarillo Bone and Joint are dedicated to providing you with the best possible care and service, and regard your understanding of our financial policies as an essential element of your care and treatment. To assist you, we have following financial policy. If you have any questions, please contact us at the numbers below, based on the need you have.

If you have a question or discrepancy with a bill or statement please have your bill, statement, and insurance information available, and call ‍806-468-9700.

If you have a question about a payment plan that you have set up with us, please call ‍806-468-9700 ext. 4005.

Insurance Filings

You, as the responsible party, must furnish our office with the up-to-date insurance information. A copy of your insurance card will be made. Report any changes in insurance coverage immediately upon arrival. Any balance due, not covered by insurance, is your responsibility and is due upon receipt of statement from our office.

Self-Pay Patients or No-Insurance

If you have no insurance and/or have experienced hardship, ABJC has developed an affordable policy in which you can utilize:

• An office visit in which an x-ray is needed is $250.
• An office visit in which an x-ray is not needed is $200.
• There may be additional charges after being seen by your physician. These charges will be provided to you upon check out and payment plans are available. These additional charges could include casts/splints/injections/additional x-rays/procedures.

Past Due Accounts

Accounts not paid in a responsible manner will be referred to an outside agency for collections and will reflect on your credit rating. In the event the account is sent to collection, you will be responsible for attorneys’ fees, if applicable. Please call our Business Office at ‍800-355-1891 if you have financial issues and wish to set up a payment plan. You will be able to make monthly payments toward your balance and avoid the collection process.
