New Patient Paperwork
There are several ways to request an appointment with the physicians of Amarillo Bone and Joint. You can call Amarillo Bone & Joint at (806) 468-9700 and have one of our admitting personnel assist you.
To facilitate the appointment process, please have your insurance card available as we will ask you about your coverage.
We require that you bring any recent X-Rays, MRIs or CT scans with you to your appointment. You will have to contact that facility where the procedures were performed and sign a release for the films. We would like to have the film and reports. Without those films, we may have to reschedule your appointment so that we do not duplicate diagnostic tests. In some cases, the films or techniques are not adequate and may need to be repeated.
Also, if your insurance plans have office visit co-pay, you will be expected to pay the co-pay at the time of your visit.cm
